Step 1  Click Book Online.  Pick a date and time for your appointment.

Step 3  You will be emailed an intake form(s) to fill out before your appointment.  Then a Licensed Board Certified Physician will reach out to you via Telemed-video or phone.  Once approved you will be able to go to your choice of Pharmacy.
